bad breath

hi i am 27 yrs female i am having bad breath which is very offensive. i brush my teeth twice daily .my tounge is coated white. my teeth are also yellow stain from inside.i hve taken medicne like merc sol , ant crd , arnica but with out any results .kindly advice.
