Obstructive Jaundice

About an year ago a 70 year old person was diagnosed with obstructive jaundice and was operated upon to place a stent. Now his bilirubin count is up again and he is feeling weak.

What are homoeopathic options for obstructive jaundice in the first place and his present condition?

Obstructive jaundice is caused by obstruction of the bile ducts, with gallstones.

The liver normally produces about 1 litre of bile each day, which is secreted (passed) into the bile duct system and stored in the gallbladder. The bile duct empties into the upper intestine (duodenum) to help in digestion. Obstruction anywhere in this drainage system causes the blood levels of bilirubin to increase, resulting in 'obstructive jaundice.' Common causes of obstructive jaundice include gallstones and tumors of the pancreas or bile duct. With gallstones, jaundice may be intermittent if the stone is not firmly stuck. Rarer causes of obstructive jaundice include parasites such as worms, scarring from previous surgical procedures, bile duct inflammations, and, in infancy, congenital malformations of the bile duct system. Additional symptoms of obstructive jaundice include dark urine, pale feces, and itching, although there is no pain. Sometimes the cause of obstructive jaundice is cancer (malignant obstructive jaundice), in which case treatment is by chemotherapy, radiation, and/or biliary drainage (surgery is rarely used). Obstructive jaundice is often accompanied by severe irritation of the skin ('pruritus'), due to a buildup of other bile constituents (salts) in the blood. In addition, because the bile is not entering the intestine, the stools will be unusually pale in colour and may be bulky and smelly. This is called steatorrhea.


please let us know the actual cause of the disorder and also the symptoms related to digestion, stool, urine etc.

the remedy can be found out based on the etiology and presenting symptoms.

Please confirm your symptoms as advised by dr. rishimba. However, CHELIDONIUM MAJUS-Q is the best remedy in such case.

Chelidonium is not a cure all in all jaundice cases.

Chelidonium is the real remedy which can be used safely and trustly in such cases. The reference given by you is not an authority to qoute.However, other remedies according to symptoms can be used.

In the reference mentioned a mother is telling that Chelidonium failed to give any response when given to her daughter.

There are many remedies for jaundice and it is not correct to prescribe chelidonium for each and every jaundice case blindly, without knowing the characteristic symptoms of the patient.

Here is the medical report about the pathological cause for the obstructive jaundice:
Clinical Details:
Selurosing cholangitis
? Cholangio carcinoma
(Hilar stricture)

Specimen:
CBD brush cytology

Microscopic description:
Loosley cohesive clusters of mildly pleomorphic cells. Some are forming papillae. Cells have scanty cytoplasm vesicular to hyperchromatic nuclei. A few cells are dispersed. Nuclear ovelapping is seen. Many cells are degenerated. Some cells have a single nucleolus. Background contained number of inflammatory cells mainly neutrophils. Small number of benign ductal cell clusters are seen.

Impression:
Smears positive for malignant cells
Morphology is of Adeno carcinoma.


He has this problem since June 2006, after the stent was put ,the bilirubin count came back to normal.Considering his age and strenth,they didn't give him any medicines or chemotherapy.But he had been using ayurvedic medicines, to which he responded positively.He gained weight and general condition improved.Recently he got fever,which wouldn't go so gave blood for examination in which the bilirubin count has increased to 3% and the report says he has sluggish liver.

Are there any medicines for this condition?Please help.

It is a case of the stent too getting blocked due to the cancerous growth.

The pathological reports help to the extant of determining the prognosis, which seems to be poor for this case.

Please wait for Rishimba for any possible advice.

I think a clarification is in order. The medical report is from 2006. Based on which a stent was placed. I dont think the stent is getting blocked. His current blood work shows elevated bilirubin levels as reported earlier.

One of the probable reasons for the bilirubin levels going up is the stent's blockage.

There may be other reasons too. I know that stents too get blocked over a period of time, and I know cases where restenting was done.

a thorough investigation is required to know if there was an infection around the stent which manifested in a fever.

there have been cases where the stent is not taken well by the body leading to some epithelial growth around it amounting to some obstruction.

homeopathically, the pathology is most important to be studied resulting out of this irregularity.

but in this case, i would advise the patient to take a scan and let the acual cause be found out. probably, re-stenting is one of the solution, if there is no additional tissue growth around the region.

because of lack of symptoms and the presence of the stent in the system, a homeopathic cure is recommended at this stage.

Thank you Dr. Rishimba. Patient is getting lab work done incl scan. Will post the findings for further advise.
