Hair loss

I am a 17 year old male. I am experiencing hair loss from the last 5 months.. I have dandruff since a few years and lately it has got even worse. I have a habit of scratching my head and pulling of dandruff flakes which is many time accompanied with a hair strand. the hair loss is spread throughout and the head and hair density is less at the sides and at the top of the head to some extent.

Also my dad and grand dad started balding at the age of 40- 45.

Please help me.. Im having problems concentrating because of this problem
