Help for my dog

I have worked briefly with two homeopaths to help my dog, but didn't get much support from either of them.

My dog is anxious and fearful of being approached by anyone, people or dogs...if they are focused on him. If they're just walking by he's fine. This most often happens on leash. He is extremely sensitive to everything...noise, wind, my emotions when I'm walking him, and especially eye contact from other dogs. He locks eyes and gets a crazy look in his eye and you can see him loading with adrenaline then he flips a switch and will even snap at me for restraining him by the leash (he is currently wearing a muzzle when I walk him in public). Always on high alert. He had Babesia when I got him and probably still has it to some degree (a tick borne blood parasite that lives in red blood cells). He is very sensitive in his throat, and sometimes gets swollen glands that obviously make it hard to swallow and he sometimes drools a little. His behavior is much worse when he has this. He also has stomach problems, no appetite early in the day then very hungry in the evenings. He was first treated with Lachesis LM 1 then LM 2 but I wasn't thrilled with the vet so stopped working with her before I was able to figure out if it helped. Another homeopath suggested Lyssin, which I gave him twice, 30c then 200c a month later. He did well on that but about 10 days after the 200c his stomach got worse and he developed terrible breath and didn't want to eat much, and the behavior came back. He had never had this kind of bad breath before, it smelled like poop. He also has brown stains on his teeth and a lot of decomposing of his teeth for his age (4)...they look like the teeth of an old dog.
When he lashes out I am able to stop him relatively quickly and then he acts like he's very upset and regretful...becomes a submissive little lamb.
