Please answer the following questions in a descriptive manner after careful analysis and recollection of previous experiences and happenings.

1. Describe your main suffering?
I am mainly suffering from pcos for past five years,I have taken lots of allopathy medicines but no use. My weight is also gaining desparetly as I am in hostel I cant follow the diet supplements.
Lot of times I am feeling sleepiness. I am not active now days,my memory is also decreasing.My skin looks so dull and looks like aged one.
2. What other physical sufferings do you have in your body?
Lots of hair in my chin and in my body also.i feel guilty to face other girls. nowadays I am afraid of lot.
Sweats lot and also urinary gland is irritating while urine comes.
3. What mental sufferings / feelings do you have associated with your physical sufferings?
Sometime I feel to die that I am not looking like previous I lost my structure and also look.

4. What exactly do you feel when you are at your worst?
I feel to die, and if I got anger I throw something. Nowadays I feel frustrated as I have many family problems.
5. When did it all start? Can you connect it to any past event or disease?
No disease in past but my lover left me from that onwards I started hating everything and also family faces economical problem in worst situation.we lost all propertys.
6. Which time of the day you are worst?
Afternoon I feel lazy in winter also.
7. What are the things which aggravate your suffering and which are those which ameliorate the same?
Pcos and weight gain, my skin looks dull and I am not active.
8. Do your think your sufferings have relation to any external stimuli (like, change of place) or any internal biological changes in the body, like, menses (in females)?
No pcos is existing for past five years, now only I shifted to other place.i am alone here I want to be always with my family.
9. When do you feel better, during hot weather or cold weather, humid or dry weather?
Cold weather is ok,but in winter also I have lot of sweat and even in hot I face some cold problems.
10. Describe your general mental set up? Are you Moody, Arrogant, Mild, Agreeable Changeable, Nervous, Suspicious, Easily offended, Quiet, Arguing, Irritating, Lazy etc.

- How do you feel before or during a thunderstorm?
After thunderstorm I will afraid.
- Do you like being consoled during your tough times?

- Are you sensitive to external stimuli like smell, noise, light etc?
yes
- Do you have any typical habit or gesture like nail biting, causeless
weeping, talking to one self etc?

- How do you feel about your friends, family, your children and especially your husband / wife?
Good friends,I love my mummy very much.

11. What are your fears and do you dream of any situation repeatedly?
I am getting devil dreams repeatedly.

12. What do you crave for in food items and what are your aversions?
I like spicy chaat items

13. How is your thirst: Less, Normal or Excessive?
excesive

14. How if your hunger: Less, Normal or Excessive?
Normal

15. Is there any kind of food which your body can’t stand?
Brinjal

16. Is your sweat normal or less or more? Where does it sweat more: Head, Trunk or Limbs?
More in head, trunk and limbs
17. How is your bowel movement and stool type?

18. How well do you sleep? Do you have a particular posture of sleeping?
I sleep by 10.00 p.m and I woke by 5.30 A>m. No in all postures I sleep.

19. Do you think you are able to satisfy your sexual desires in general?

20. How do you think you are different from others, if at all?
My work style is different and will be honest

21. What medications have been taken earlier by you to treat the diseases and do you have any particular symptom surfacing after the medication?
I am taking AGNUS CASTUS Pentarkan

22. What major diseases are running in your family?
No major disease like that

23. Describe, how do you look like? Describe your overall appearance
I am short,whiteash,fatty girl with long hair and round face.

Please answer the following questions:
(Please give details of your past menstruation if you have attained menopause.)
- Are the periods early, regular or late in general? How long do they last? If I take any English medi then periods will come.otherwise no
- Do you suffer from any kind of physical or mental discomfort before, during or after the periods?
Yes back pain is there and also sometimes feel irritate
- Is the flow scanty, normal or excessive? Normal
- Is the blood thick bright red or pale watery? Thick bright red
- Do you notice any clots in the flow? yes
