This is just a forum. Assume posts are not from medical professionals.
Ok, we need to do a chronic case assessment then.

You might repeat some of what you said before, but I would like it all in one place, and you may actually rephrase the information in a way that is useful to me.

MENTAL STATE OF CHILD


1] What is the effect of main complaint and associated complaints on him/her?

2] What are the thoughts/feeling/reactions associated with it? Describe in detail.
3] Any unusual sensation in their body. (Describe the sensation they experience during all stressful situations like nightmares, fears, before exam, with the incident, which had a deep impact on him/her.)

4] What are his/her fears (existing and/or imaginary)? What are the feelings/thoughts and the reaction associated with it?

5] Any incident which had a deep impact on him/her? Describe in detail. What are the thoughts/feelings/sensations associated with it? At that moment of time what were his/her feelings/thoughts, sensations and reactions associated with it?

6] What are the stories/fairytales that he/she likes to read / listen? What character attracts him/her the most and why? Describe about HIS/HER understanding of the stories. What are the feelings/thoughts associated with it?

7] What are his/her imaginations/fantasies? Describe in detail.
8] What are the dreams that he/she gets? What are the feeling/thoughts and reaction associated with it?

9] What are the nightmares that he/she gets? What are the feeling/thoughts and reaction associated with it?

10] What are his/her interests and hobbies?

11] Describe about the specific toys, games/specific TV serials, cartoon characters, movies he/she likes. What are the thoughts, feelings associated with it? What kind of questions does he/she asks related to that?

SLEEP PATTERN

1] Describe the posture in sleep. (On the back, side, abdomen etc.) Any particular position in which he sleeps? In which position he can’t sleep?
2] During sleep does he /she:
a) Snore?
b) grind teeth?
c) Dribble saliva?
d) Sweat?
e) Keep eyes or mouth open?
f) Walk? Talk?
g) Moan? Weep?
h) Become restless? Wake up with a jerk?

APPETITE AND THIRST

1] How is his appetite?

STOOL

6] Does he/she have belching or passing of gas? Describe its character.
7] How does he/she feels after passing gas up or down?

Urine and urination
1] Any problem about urine?
2] Any strong smell? Like what?
3] Does he / she has any trouble before, during and after passing urine?
4] Any difficulty about the flow? Slow to start, interrupted, feeble dribbling etc.?
5] Any involuntary urination? When?


SWEAT/PERSPIRATION-FEVER-CHILL
1] How much does he/she sweat?
2] Where and on what part does he/she sweats the most?
] Does he/she perspire on the palms or soles?
4] Is the sweat warm, cold, clammy, sticky, musty, greasy, stiffens the linen etc.?
5] What is the smell like? E.g. foul, pungent, sour, and urinous.
6] What color does it stain the clothing?
7] Is the stain easy to wash off or difficult?
8] Any symptoms after sweating?

9] How does he react to hot/cold weather and monsoon?
10] When does he get fever or chill?
11] What brings it on?

CHEST-HEART – COLD – COUGH
1] Does he/she catch cold often? If so, how often?
2] Describe the symptoms, nature of discharge etc.
3] Is there any trouble in his/her CHEST or HEART?
4] Is there any trouble with his/her voice or speech?
5] Is there any difficulty in breathing?
6] Does he /she has cough?
7] Is it more at any particular time?

I suppose nighmares. Night terrors. Crying while sleeping.

Unfortunately I dont know.

He may be afraid of the dark but I always leave a light n for him so that's not the cause of the night terror.

5] Any incident which had a deep impact on him/her? Describe in detail. What are the thoughts/feelings/sensations associated with it? At that moment of time what were his/her feelings/thoughts, sensations and reactions associated with it?
He has separation (from his parents) anxiety.

6] What are the stories/fairytales that he/she likes to read / listen? He likes Sesame street, not old enough for fairy tales.
What character attracts him/her the most and why? He likes Elmo and Mickey Mouse.
Describe about HIS/HER understanding of the stories. What are the feelings/thoughts associated with it?
He doesnt follow stories yet. But he likes characters.

7] What are his/her imaginations/fantasies? Describe in detail.
8] What are the dreams that he/she gets? What are the feeling/thoughts and reaction associated with it?

9] What are the nightmares that he/she gets? What are the feeling/thoughts and reaction associated with it?

10] What are his/her interests and hobbies?
Loves music

Stacking toys, card, mega blocks, strollers.
Sesame street, Mickey Mouse.

Too young for sports but he is very active, climbs, runs ..

Sweet, smart, very interested in the alphabet. Already understands 3 languages.

16] What does he/she wants to become when he is grown up and why? What are his/her ambitions?
17] Whom does he/she idealizes and why? What about him that he/she admires the most?

Loves his parents and grandparents, very sweet, loving.

Being startled makes him laugh, silly behaviors make him laugh too.

22] What makes your child very angry and irritable?
When you take something away from him.

23] What does the child do when he/she is alone?
Not happy

24] Is there any particular reaction does he / she throw about a particular person?

25] Have you observed any change in his/her behavior on starting a particular T.V./radio program? If so, what is it? How does he/she react?
He starts dancing when Barney, a kid's show, starts on tv.

He sleeps on his back, side AND belly, havent seen a preference yet.

He prefers salty. Loves white yogurt. Does not like mango.

STOOL
1] Does he have any problem regarding stools? Not usually, but yesterday he ate tons of raisins and got an upset stomach.
2] When and how many times a day does he pass stools?
2/3 at least.
3] When is it urgent?
4] Does he /she have any problem about bowel movements? No
5] Does he/she have to strain for stool? Even if soft? No
6] Does he/she have belching or passing of gas? Describe its character. Has gas, after he eats/drink he belches, during the day he may pass some gas.

Normal.

As I started to look through these questions, I realised there is almost no case here at all!

You will need to try to give me more details.

What exactly does he look like when he is crying?

What does he do while you are holding him?

Does he wake up at all?

When he wakes what does he say?

What things is he afraid of? It doesn't matter whether it is at night or not - anything at all.

What is he like with you? How do you notice separation anxiety? What makes it better? What makes it worse?

Is he the same with his father?

What is he like on his own?

What makes him feel better when he is upset? What makes him worse?

What was the pregnancy like for you? Any problems? What was your emotional and mental state like during this time? Were there any traumas or other significant events?

How was the birth? Any problems? Again, what was your mental state during the birth like?

Were there any other significant events in his life over the last 18 months?

What exactly does he look like when he is crying?
His eyes are closed, he cries with tears, he calls for mom or dad or both, sometimes he screams. He is asleep but sometimes he ooen his eyes.

What does he do while you are holding him?
He pushes himself back, away from me, he does not put his head on my shoulder, he does not calm down, he cries for mom or dad he is very tense and tries to 'get free' ...

Does he wake up at all?
He opens his eyes but he is still sleeping.

When he wakes what does he say?
He calls for mom and/or dad.

What things is he afraid of? It doesn't matter whether it is at night or not - anything at all.
He doesnt seem to be afraid of anything yet but he is very shy with other kids and ppl he doesnt know.

What is he like with you? How do you notice separation anxiety? What makes it better? What makes it worse?
Recently he got closer to me, he used to always want dad but now he wants mom too. He is very sweet with me.
When I leave the room he cries 'mom!'. Keeping him entertained helps sometimes.
If I dont come back right away he keeps crying, but depends on his mood.

Is he the same with his father?
Yes.

What is he like on his own?
He can play on his own, used to but recently he always want me to play with him.

What makes him feel better when he is upset? What makes him worse?
Hugs, rocking, sushing.
Ignoring him makes it worse, not giving him what he wants makes it worse.

What was the pregnancy like for you? Any problems? What was your emotional and mental state like during this time? Were there any traumas or other significant events?
Pregnancy was totally fine.
I was super happy the whole time, took me a year and 9 months to get pregnant, nothing bothered me, I was way too happy to let anything bother me.

How was the birth? Any problems? Again, what was your mental state during the birth like?
Birth was 1 week early because water broke but went well. They gave me pitocin (which I had no idea what it was at the time) ,,, at one point his heart rate dropped but was able to push hard enough to get him out quick.
My mental state was very calm, my husband was actually amazed, I waited a looong time to have this baby and I am talking about almost 20 years, I wanted to have a baby since I was 17 but got married late.

Were there any other significant events in his life over the last 18 months?
The most 'significant' event involves something that is taken away from him, usually my phone. That causes a major tantrum. Nothing else. But I have to admit that I lost my patience a few times and yelled at him and I feel extremely guilty abt it. But he has been having the terrors since before I yelled .... He has been having them already for a year or more but not every night. I try to notice what he does during the day that might trigger it but havent been able to point out the cause, if there is one.

Forgot to mention that when he was 1 week old there was a major hurricane here in N.y that left us with no heat, electricity or hot water .,, we stayed for a week then we went to family. Not sure if that may be considered a 'significant event'.

Why do you leave the light one when he sleeps? You said you don't know if he is afraid of the dark.

He used to sleep in total darkness no problem, but I thought the terror *might* be caused from it but he has been having them even with the light on. I leave it on just in case ... Don't want to take chances but turn it off when I go to sleep.

Ok so you are saying that he wakes up afraid regardless of whether it was dark and light? Alright then.

The remedy I would think of first and foremost is Stramonium, but the lack of fear of the dark seems strange. It is an important piece of the picture for that remedy.

There are three rubrics that might apply here:

Does not recognize relatives on waking - aesc, Kali-br, stram, zinc

Clinging, awakens terrified, knows no-one - BORX, cina, STRAM

Shrieking during sleep with fixed eyes - ant-t

If I then look at the following rubrics as well, keeping in mind only those 7 remedies:

Weeping in sleep
Inconsolable/Consolation aggravates
Shrieking during sleep
Frightened on waking

The remedy that is present in all these rubrics is Zinc.

I would obtain Zinc 30c in liquid form. Give him 3 doses, one each night before he goes to sleep. Give it exactly the same way you did the Spongia. Stop if any of his symptoms aggravate.

Hi! I got the remedy but was waiting for my baby to have another night terror before giving it to him. Strangely he hasn't had a night terror since, but he had teething crying instead, so I figured that was not the right remedy for it...
Thanks so much for all your help though, if he starts having the night terrors now I know what to give him!

Do not give him that remedy unless we discuss it first. It is quite possible the Spongia has cured it, as I hoped it would.

Hi, after I gave my son 2 doses of chamomilla, the fever seems to have gone away but now his chest seems to be 'closed' (yesterday I heard some rattling in his chest but had no cough), he just had a one episode of hoarse, raspy dry cough in his sleep right now, didn't have it all day, just happened now for the first time but all day he was sad, whiney and needy, wanted me to pick him up all the time and didn't eat much. I thought the cough may be from a dry mouth/throat but it's either a cold or another teething symptom. I believe it's hard to suggest a remedy when we are not sure what's the actual issue ...

The Chamomilla may have suppressed the fever. You should give another dose of Spongia right away.
