Creatinine 2.78 bun 54 -lupus

Can anybody pls help for ckd3. Both kidneys shrinked. Diagnosed with sjorgan syndrome
Me,connectivetissue disorder c3,c4 , dsdna normal, Ana -130.
Kidneys destroyed only due to wrong medications from time to time. Having this problem from 2009
Can anybody help to bring down creatinine..
