What is the cause of the low sperm motility in your case? Do you have an infection? or undescended testicles or have you had a surgery in the past or injury or something else?
Also, are you taking any medications? Your black tongue suggests that you are probably taking something or having an unhealthy lifestyle.
It's important to let the prescriber knows all those things.
