Diabetes, Inguinal Hernia, & RLS (Restless Leg Syndrome)

Dear Doctor,
Please suggest me medicine according to the following symptoms:
Present Complaints:
1. Diabetes,
2. Inguinal Hernia, Right Side, Inside Scrotum, Painless, Round, Hard
3. RLS (Restless Leg Syndrome)
4. Impotence & aversion to sex
5. Doesn't feel sexual excitement on wife but when he sees porn he feels some excitement. Feels very guilty on this behavior
General Symptoms:
1. Extreme fear of darkness / ghost esp. when alone at home
2. Severe thirsty for large quantity of cold water
3. Always drink COLD water
4. Extreme lazy & aversion to work.
5. Filthy & dirty
6. Does 2 or three time bath during hot day but in winter does bath after two or three days
7. Profuse sweat
8. Hot patient (like loose garments & do not like garments during sleep)
9. Must be fanned
10. All discharges are offensive
11. Great loss of memory & difficult to concentrate
12. Desire egg, milk, sweet, travel, open air, river,
13. Dream: Falling from high, Passing stool & urine, freight.

Past History:
1. Bite of rabid dog & took injection
2. Piles operation

Family History:
Mother- Diabetes, Heart stroke to paralysis, Brain stroke just before passed away.
Father: Diabetes, Passed away by Heart stroke.
