I do have faith in homeopathy that's the reason I reached out.
While we have learnt to accept and love him the way he is. We do want him to be more peaceful and his social acceptance in his workplace may improve if his anger and tendency to talk loudly to self reduces.

Please answer the following questions.

1. I understand his intellect is weak since the beginning but I need to know since when is he talking to himself? What was going on in his life when you first started noticing him talking to himself?

2. Does he talk obscene things/words as well?

3. Does he talk to himself lying on his bed at night or in any other position mostly? Does his 'muttering' increase as soon as he lies down on the bed?

4. Is he OK sleeping in the dark? Or does he want lights to be on always?

5. How do his mental symptoms change in the dark?

6. Does he listen to voices? Voices that only he seems to hear? I mean deliriums and delusions.

7. What does he talk about mostly? Is it about the people he meets during the day? Can you record his 'talking to self' and send the file to me through email?

8. Has he ever admitted having hallucinations?

9. Does he talk in his own mother tongue or in some other foreign language?

10.Does he sweat more than average? Is his skin mostly moist due to sweat?

11. Does he ever talk about sexual matters? How is his sense of shame? Does he follow general social rules of covering private parts always?

12. Why does he not wash himself? Is it because he cannot stand the sight of water? Have you ever asked him this question?

13. How do his symptoms change after meals?

14. Does he ever admit that he is possessed by two persons or wills?

Am replying in order

1. I understand his intellect is weak since the beginning but I need to know since when is he talking to himself? What was going on in his life when you first started noticing him talking to himself?
I started noticing mainly after my mother’s death, since till she was alive she used to take care of him. However I am told that he used to talk to himself since his late teen, since he had very few friends and used to be bullied, so he used to create his own day dreams to speak his thoughts.
Though his intellect is weak, he has a fair idea of how he should be treated, as he is elder to me. He also has his own thoughts on what is right /wrong, particular about his likes and dislikes. Though he collects a lot of unwanted things and hides in his room, he is also very particular about ensuring that he wears properly ironed clothing.

2. Does he talk obscene things/words as well?
Typically no. Sometimes, his talk is also around reliving happy memories. Though if he is really angry at someone then he does use expletives/ abusive words against them, which in real life he would not dare to say. Me and my father have been few of the people against whom he has had a lot of anger since we both did not understand him as a different child and had bullied him. Though at a later stage I did understand his special needs but by then the bond had snapped. Now since the last few years I am working at it, and he seems to be opening up to me.

3. Does he talk to himself lying on his bed at night or in any other position mostly? Does his 'muttering' increase as soon as he lies down on the bed?
No. His talking to self can be anywhere he is alone, sometimes, even with others in the room, if he gets to sit in one corner or no one is paying any attention he can be seen talking to self.
On bed it doesn’t increase, in fact if he is upset or angry, he doesn’t lie down, keeps pacing the room or maybe sitting. However, if forced to go to bed, when he is angry, he keeps muttering even with eyes closed.

4. Is he OK sleeping in the dark? Or does he want lights to be on always?
He is perfectly fine sleeping alone and in the dark.

5. How do his mental symptoms change in the dark?
I haven’t noticed any significant change.

6. Does he listen to voices? Voices that only he seems to hear? I mean deliriums and delusions.
No. This was thoroughly checked at NIMHANS. It is absent. What they said is that since he doesn’t have many friends or social interactions to express his feelings, this talking to self for him is a way to express his thoughts the way he would ‘really’ want to. So if he wants to shout at someone whom he can’t shout in real life, he uses this as the media, or to relive old memories and reconstruct them the way he would want.

7. What does he talk about mostly? Is it about the people he meets during the day? Can you record his 'talking to self' and send the file to me through email?
Typically about people in his office. It also mainly involves his anger towards my father and me, his previous classmates and our social friends circle. Sometimes it also involves my mother.
We tried recording but somehow we have not been successful as he keeps moving/ walking when talking and hence the recorder doesn’t catch the sounds.

8. Has he ever admitted having hallucinations?
Nope. This too was thoroughly checked at NIMHANS by keeping him for a few days at the NIMHANS. It was found to be absent.

9. Does he talk in his own mother tongue or in some other foreign language?
Language varies between Mother tongue and hindi which is our vernacular. He is not very good in English. Though if someone had said something to him in English, specially abusive words, then he repeats them.

10. Does he sweat more than average? Is his skin mostly moist due to sweat?
Yes

He doesn't speak about it, but he does feel attraction to pictures of female models in Newspapers. He does follow the social norms of covering his private parts properly. Also he is very shy about his private parts and never opens clothes, not even his shirt, in front of anyone else. 3 years back there were a couple of incidents in his school when he had commented on some females, possibly innocently but no such incident had come to light since then.

12. Why does he not wash himself? Is it because he cannot stand the sight of water? Have you ever asked him this question?
He does wash himself regularly. He rarely skips taking bath. Though he is not very focused on cleaning, scrubbing, using soap etc. To us it seems like a of training and lack of reinforcement on personal hygiene. He doesn't seem afraid of water. He likes playing with water.

13. How do his symptoms change after meals?
During meals he takes a long time, since he manytimes seems to be in his dreamworld. If we do not put a time limit to finish meals he can easily take 50-60 min for a meal. Though I haven't noticed any specific change after meals, typically after meals me and my husband either get busy with our work or go to sleep, so he is left alone, and then again he goes into his self-talk.

14. Does he ever admit that he is possessed by two persons or wills?
No never

Yes he did. This was long before my marriage, one year after my mother's death. He was extremely attached to mother, she was the only one who could understand him and sincerely cared for him. During that phase we were going through a rough financial time due to my father losing his business because of a property dispute. Home environment was extremely tensed. Also we were trying to get him trained in computers, where again his teacher was a bit rude. During that time he had tried walking into a pond, but came back home because he got scared. He did not tell us, but from his wet clothes my father could understand and after a little bit of questioning my father understood. Much later, he actually admitted it to the psychiatrists at NIMHANS almost 3 years back.
After talking to him extensively Nimhans doctors said that it was more due to the grief and not a likely pattern.
In the last 6 years that we have observed him closely, he has never said that he wants to die.

The answer to q16
usually repitive themes around some incidents! sometimes appears to have a mental dialogue or reconstruction of the incident. Though difficult to understand the exact dialogue. And at times lot of anger and sometimes happy tones.
Not all words are distinct but it is not unstructured mutterings. Often keeps repeating the same sentence multiple times.

Let him take a few doses of ANTIM TART 30C initially every 4 to 6 hours till his cough symptoms change or subside.

Once his cough subsides, taper down the doses to two a day.

In case new symptoms arise, or his cough changes form, let me know.

Once he is cured of acute symptoms, we can try out his constitutional remedy covering his mental condition.

When he takes Antim Tart 30C, please take a note of his mental symptoms also.

Share notes after a week.

Hello Rishimbaji
I have given my brother the 1st dose of ANTIM TART 30c. Morning he was having watering through nose, now that has stopped but he says his nose is blocked.
Though he is not coughing but he is having to clear his throat. Also an hour back I had checked his temperature it was 99, now 10 min. Back it touched 99.5.
Is this part of the healing??
Should I give him the second and third dose for the day as you suggested???

Please give him some 4 to 5 doses for a day or two maximum. But if he starts feeling better or worse, stop the doses.

Mild temperature is not worrying, its a body response to the infection and considered a part of healing.

Note the color of the sputum and tell me if its rattling in his chest.

What is the quantity of phlegm? Which side is the blockage worse...right or left?

Quantity is more when it comes out. Which side is the blockage worse...right or left?
Blockage is worse on the left side.

On the mental symptoms side,he seems very cheerful n has not been talking too. My husband and him kept cracking jokes with each other. Earlier, if he had cold n cough, he used to be very cranky...so this is actually a pleasant change for us.

Let us wait for some 3 or 4 days and see if he gets over this bout of cold.

Once he is free of cold and cough, I will suggest his constitutional remedy.

If this cold doesn't get over in the next 4 days and changes form for the worse, let me know the new symptoms so that another remedy can be suggested.

Stop ANTIM TART.

With two days of doses his cold n cough seems to have come down substantially, should I continue to give him ANTIM TART, stop it or bring it down.
Last two days have given him 4 times a day.

My brother's cough n cold seemed to have come down yesterday but today morning he was coughing again and phlegm was coming out of his nose too.
Would you suggest that I give him the medicine again or just let it be??
My only worry is that we r going out on a vacation for a week, it's too late to change plans plus he too is very excited about it.
Would be great if you could share your thoughts.

He seems to need to clear his throat, and a slight cough. Nose too phlegm is coming out.

The above two. Though his anger seems to be less, but he is talking to self when alone.

In the cough its whitish, from the nose greenish.

Loose and quantity is more

Early mornings and nights here are damp and cold which makes it worse and daytime when it is bright and sunny, he is better.

-How is his thirst and hunger?
Normal

Can't say
-If nostrils are blocked, which side is worse?
He had mentioned left side

-Any problem in talking?
No

Dear Rishimbaji
We came back from our vacation today.
I had given my brother Kali Bich 30c as suggested by you on last friday and saturday.
His cold is gone, the cough too seems like the last vestiges of it, with some sputum, which is white and loose. Not rattling in the chest.
However, during the trip, he was extremely agitated. He was excited about the trip but got angry I believe due to me asking him, not to swallow the sputum. This happened on the day one just when starting. Since then, he was angry about one thing or the other.
Though on one hand he enjoyed a lot, clicked many pictures but still kept on talking to self.
He used to keep talking to self throughout the day and even till late in the night. Many nights he would not be able to sleep till 2:30 or 3 am due to his talking. When travelling we three stay in a family room, so I was able to notice. Though his voice is usually low when he is speaking, however he tends to speak loudly when very agitated and his tone seems to change with more roughness in it.
The themes were:
* how he wouldn't have had to come to stay with us if not for my father's health.
* about my father's death due to ill health and possibility of his longer life if he had stayed in our hometown
* he will not die like my father due to ill health
* and thoughts about going back to our home town.

Do share your thoughts.

If you think we should start with his treatment, then give him the following dose-set on a single day.

LYCOPODIUM 200C three doses, each dose 10 minutes apart.

Give him these doses in empty stomach and let him not take any food or drinks for the next two hours. So, it is better to do it on a weekend morning.

Take notes on his mental symptoms for the next one week and share with me here.

If there is a response, we need to continue with this remedy in continually increasing potencies till he is completely fine.

The lycopodium seems to have done a miracle.
The day-wise details of the response in the last week are shared below:
Day1: Saturday (was a state holiday so was home)
The medicine was given in the morning. Allowed him to sleep till late. He woke up fine, was very quiet throughout the day. Even in bathroom, he finished his bath on time and there was no sound of him talking to self.
Post-lunch when he was alone, he was talking to self for sometime, but the sound was not audible, and stopped within a few minutes.
Rest of the day remained extremely quiet. However, he did not show any enthusiasm about anyting, nor did he smile at all. Did whatever was asked silently, and gave clear answers and opinion whenever required.

Day 2:Sunday
Throughout the day was silent.
Did whatever was asked silently,was not smiling at all. Gave to-the-point and clear opinion about things when asked. For example, when we asked to go for a walk in the park, he refused saying its too cold outside.
Though he was quiet, he seemed to be thinking deeply, though no self-talk. And throughout the day was not smiling at all, even at jokes or while watching TV.
Thankfully, by evening I observed him enjoying a movie on TV and he seemed to be relishing it.
Night, when he was alone, he was talking to himself for a few moments, then became quiet and went about his pooja quietly. Went to sleep much before his earlier time and slept well.

Day 3: Monday

Overall was quiet, went about his work quietly. Got ready and went to school on time, without me having to nag him.

Evening, after coming back from school, seemed peaceful and shared briefly about his day at school.

Dinnertime, was happy and shared his happiness about the last week's vacation.

Post dinner, when we all went to sleep. for a few moments was talking to self, but in very low voice then became quiet on his own and did his night time pooja and went to sleep. Slept peacefully.

Day 4: Tuesday
During the time he was home, he was peaceful. Managed his time well.
Dinnertime was happy and together we recounted last week's entire trip.

Day 5: Wednesday
Was peaceful throughout the day and evening too was good. Before dinner I noticed that his office chappal had broken and I suggested that next day I'll go to his school, he can take half-an-hour off from school so that we'll buy a new one. he objected to it and insisted that we do it after his office time, once he comes back home. That incident seemed to have briefly disturbed him and then he was talking to himself for sometime. Later we all got together and spoke about various things. He seemed fine and cheerful.

Day 6: Thursday
During the time he was home, he was peaceful. Spoke well and answered whenever any questions were asked. Clearly expressed his opinion and preferences whenever required. Managed his time well.
Dinnertime was quiet but peaceful.On his own shared a few incidents from his office.

Day 7: today:
Morning was peaceful and went on time.

This last week I also noticed that he has been wearing neat and clean clothes without me having to ask him to do so.
When he doesn't like something, he states it clearly without any anger or irritation.

So it does seem that the medicine has created a response. Do suggest the next steps.
