I am a firm believer in memetics and feel that each of us in our own little way can contribute to reshaping a new reality.This begins by stating emphatically what we expect a certain component of say medicine to be like in the future etc..The human race in general does a remarkable job of following instructions.Even the powerful succumb to popular vote and fall in line in a manner of speaking.
